Home-Based Programs

Service Description

We provide personal care, meal preparation, and light housekeeping to frail, at-risk individuals, enabling them to remain in their homes as long as possible, delaying or eliminating the need for institutionalization. We rely on referrals from other agencies, friends, and family members to seek out those in need of in-home services. A plan of care is appropriate so a nurse from our agency can meet the members appropriate needs. The services may be available at little or no cost to the members, depending on the guidelines of the program. We have a case manager and registered nurses to manage the programs. Available in-home programs include: - Personal Care (Medicaid) - Aged and Disabled Waiver (Medicaid) - F.A.I.R. (Family Alzheimer's In-Home Respite) - Respite - Veterans - Case Management
